Join multi-line SMTP responses with CRLF instead of LF

SmtpStream previously stripped the CR from each line of a multi-line
SMTP response and joined the lines with a bare LF, so
SmtpResponse.Response (and SmtpCommandException.ResponseText /
SmtpProtocolException.LastResponse) used Unix line endings even though
SMTP uses CRLF on the wire.

Continuation lines are now joined with CRLF. The final line still has
no trailing line ending, and lines terminated by a bare LF are
normalized to CRLF, so consumers can always rely on CRLF separators.

ParseExpandResponse now splits the EXPN response on CRLF accordingly.

Updated the affected unit tests and added a Breaking Changes entry to
the 5.0.0 release notes.
