* docs: show the filter options in the example usage

The example now passes every --filter-* option with the thresholds var2vcf_valid.pl applies by default, and a second example for pileup mode (-p) adds the labels for the VarDict rules -p switches off, at VarDict's defaults.

* docs: move the filter table below the examples and shorten the --fisher note
