feat: add the LOW_QMEAN filter (#128)

LOW_QMEAN, from --filter-low-qmean, labels a call whose ALT reads' mean per-read variant base quality (FORMAT QMEAN) is below the threshold. VarDict normally drops such calls itself with -q, but in pileup mode (-p) it keeps calls that fail its own rules, so this label marks them. var2vcf_valid.pl applies the same rule as q22.5, described as "Mean Base Quality".
