feat: add the HIGH_MEAN_MISMATCHES filter (#124)

HIGH_MEAN_MISMATCHES, from --filter-high-mean-mismatches, labels a call whose ALT reads average more substitution mismatches (MEAN_MISMATCHES) than the threshold, a sign of misaligned or paralogous reads. var2vcf_valid.pl applies the same strict comparison as NM5.25, though its description says ">=".
