Record that all five packages now carry the coverage floor (#690)

django-bootstrap3 (zostera/django-bootstrap3#1158) and django-bootstrap4
(zostera/django-bootstrap4#907) were the last two without `fail_under = 100`.
With them, `[tool.coverage.report]` is byte-identical in all five repos, so it
is no longer a per-package substitution. Move it out of that list and say what
the block is, why the floor rather than a lower number, and how to check a
floor can actually fail.

Also state the expectation in AGENTS.md, which had nothing about coverage.

Follows #688 and #689, which did the same for marina and django-icons.
