fix: Make every declared asset reachable

- fix: Refuse an assets URL prefix with a percent-encoding.
  It passes datapages lint and gen, and then the server answers 404
  for every file under it:

  The server decodes /my%20files/app.css to /my files/app.css before
  it strips the prefix, and /my%20files/ never matches that. Allow in
  the prefix only what the error message lists: letters, digits, '-',
  '.', '_', '~' and '/'. This also refuses quotes and other punctuation,
  and accepts /.well-known/, which was refused as a dot segment.

- fix: Refuse a go:embed pattern that is a glob or a single file,
  as //go:embed static/*.css or //go:embed static/app.css.
  The server serves the pattern as a directory that doesn't exist
  and answers 404 for every asset.

- fix: Percent-encode file names in assets.Path and href.Asset.
  assets.Path("a#b.css") returns /static/a#b.css, which a browser
  requests as /static/a. A '?' in a name does the same, and a '%'
  makes the server answer 400.

- fix: Show the app's own prefix in the doc of the generated
  assets.Path, which always showed /static/style.css.

- fix: Build the asset URLs in the head of example/offline-cache with
  assets.Path, as its offline config does.
