docs: tighten header descriptions to what VarDict computes (#120)

State where SVTYPE and SVLEN are written, VarDict's 1-decimal rounding on
the per-read means, the next-base REF case in AD/ADF/ADR, and fix the
Complex clauses of MEAN_DIST_TO_READ_END, QMEAN and MEAN_MISMATCHES.
Correct HICNT, DP, AF and ALT_READ_POS_VARIES where they overstated what
VarDict does. Drop the commented-out header lines and fix wrong field docs
on TumorOnlyVariant.
