Add a `packageguard init` command that scaffolds a configuration (#266)

* Add a `packageguard init` command that scaffolds a configuration

Adopting PackageGuard starts with an empty config and a question the user
can't answer yet: which licenses should be allowed? `init` scans the
repository first, summarizes the licenses actually in use (flagging
copyleft), asks what kind of software this is, and writes a policy that
fits it - built only from licenses the scan actually found, never a
policy that just allows everything present.

Closes #222

Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com>

* Address InspectCode review findings on the init command PR

Remove a redundant using directive, a redundant explicit collection
literal passed to a params argument, redundant type specifications on
target-typed 'new' expressions, and convert a delegate variable into a
local function.

* Scaffold the warn list and risk gates from PR #262 in init

init now lists tolerated copyleft licenses under warn, optionally adds
risk-based deny rules (--risk-gates or an interactive question), includes
a commented riskExceptions example, and points to --treat-deny-as-warning
for gradual adoption.

* Document that configuration files may contain comments

The generated config from init contains comments, and users can add their
own. A spec now locks in that // and /* */ comments are parsed.

Co-Authored-By: Claude Sonnet 5.5 <noreply@anthropic.com>

* Rename the init domain code to Scaffolding

The Core code describes scaffolding a configuration, not the init CLI verb,
so the folder and namespace become Scaffolding, and InitConfigWriter and
InitPolicyBuilder become ConfigScaffolder and PolicyScaffolder. The init
command and its settings keep their name.

* Make the scaffolding types internal
