chore: speed up pre-push checks and refine PR template (#888)

# Objective

- Make the local `cargo run -p ci` pre-push check quick enough to run
before every push, without losing coverage on the full CI suite.
- Streamline the PR template so it prompts for the things that matter
without generating prose noise.
- Fix docs and changelog conventions noticed along the way (this branch
also carries the README/lib.rs/CONTRIBUTING cleanup).

## Solution

- Pre-push now runs fmt, clippy, codegen and two feature test sets: `std
<deps>` (no `all-types`, which catches dependencies on gated-off types)
and `std all-types scalar-math <deps>`, both with all optional deps.
Doc, the remaining eight sets, FMA and no_std stay in the full suite /
CI.
- Added a dedicated `PRE_PUSH_FEATURE_SETS` list and moved the shared
test-command builder into `prepare.rs`, so pre-push no longer depends on
`test_features`.
- PR template reduced to Objective, Solution and a two-item checklist;
tests/benchmarks are prose prompts in Solution.
- Documented which commit types trigger a release vs ride along, and
added a `Performance` changelog section for `perf` commits.
