fix: address decentraland-bot review findings

- Disabled no longer trusts reviewedAt alone: the subgraph also moves it on
  rescueItems, so a first approval stuck between rescueItems and setApproved
  read as disabled. A not approved collection is now disabled only when a
  curation approved it, or when it has no curation and was touched on-chain;
  a pending curation keeps it under review.
- Counts read from the same collection source as the list, so is_published
  narrows them like it narrows the results.
- Third-party collections get no status, so they no longer inflate the
  draft count or match status=draft.
- Counts are only computed for paginated requests, the only ones that
  return them.
