fix: match a workspace folder name only as a whole path segment (#1349)

* fix: match a workspace folder name only as a whole path segment

relativeToAbsolutePath checked uri.startsWith(folder.name), so with
folders test and test2, test2/test.json resolved under test, and a
root-level test.json resolved to the test folder itself. Require the
folder name to be followed by a separator.

* fix: accept '/' after a workspace folder name on Windows

The folder name was matched with path.sep, so on Windows a yaml.schemas
key written with forward slashes, like test2/schemas/schema.json, did
not match test2 and was resolved against the first workspace folder.
Accept either separator and let path.join normalize the rest.
