Key connection pools on the TLS attempts they were opened with

A pool opened for prefer while the sslrootcert file was missing (TLS
without verification) kept being reused after the file appeared, while
a new pgmon process with the same DSN would verify the chain. Add the
first attempt's sslmode and the TLS retry's to the pool key, so a root
file that appears or disappears while pgmon runs opens a new pool.

Also make the README and CHANGELOG exact about prefer: rustls reports
TLS errors the same way during and after the handshake, so a dropped
connection is never retried without TLS and a TLS protocol error always
is, and a server that does not offer TLS is used without it rather than
retried.
