Writer Word2007: Point a table of contents entry's page number at its title (#2952)

* Writer Word2007: Point a table of contents entry's page number at its title

`PAGEREF {n}`, but the bookmark around the title, and the entry's own link,
are named `_Toc{n}`. Word answers "Error! Bookmark not defined." in place of
the page number when it updates the page numbers. The reference now names the
bookmark again, as Word and LibreOffice write it.

A title made of a text run gave an entry with no text, a link with no name.
The entry now carries the text of the run.
