Fix legacy TrouSerS RSA parameter parsing

Older TrouSerS versions can encode an RSA exponent size of 3 in host
byte order as 0x03000000 without including the exponent bytes or updating
the parent parameter size.

Recognize this encoding, emit a warning, and treat the exponent as
omitted so the default RSA exponent is used. Add RSA parameter and full
identity request regression tests.
