CI: fix wiki publishing on Windows runners, clearer release tag rules

The wiki was cloned with the runner's core.autocrlf=true and then switched to false, so every page looked modified and git rm refused to remove them. Clone without conversion and delete the pages from disk; git add -A records deletions.
The release job explains that the tag must be v<major>.<minor>.<patch> (nbgv tag creates v3.0.0.0 for a three-part version.json version) and accepts '## [3.0.0]' as well as '## 3.0.0' changelog headings. CHANGELOG heading uses the Keep a Changelog form.
