perf: shrink bundle and hoist requires in default export (#32)

* perf: shrink bundle and hoist requires in default export

- ranges are plain strings split into site/global arrays instead of
  {re, scope} objects and RegExp literals that were only used for .source
- drop redundant inner ^/$ anchors, already anchored by the join
- hoist require() in src/index.js: it ran on every call (~5x faster in Node)
- extractMappedIPv4 relies on its anchored /i regex instead of a manual
  prefix check; the length/charCodeAt guard stays so non-strings still throw

Output is identical to 2.4.0 for 142,511 generated inputs across every
entry point, .regex and extractMappedIPv4, including thrown errors.

* chore(benchmark): report min+gzip bundle size

The size column was fs.statSync of src/index.js, a two-line file that only
requires the real code (101B). It now bundles each library with esbuild,
minifies and gzips it: what a browser downloads.
