* feat: let a program name spans inside its own run

`run` is one number covering everything the code did, which is fine until
that number is the one you are trying to explain. Chasing a production
tail where `run` was 6466ms while the code's own work measured 5ms, there
was no way to see which part of the wrapper around it was slow.

`globalThis.__isolated_time(name, thunk)` records how long the thunk
took, returns its value, and records the span even when it throws.
`globalThis.__isolated_phase(name, ms)` records a duration measured some
other way. A repeated name accumulates, so a span inside a loop totals.

Named spans are merged under the runner's own phases rather than over
them, so a program cannot displace `install`, `build`, `spawn`, `run` or
`total`. A program that names nothing reports exactly what it did before,
which is asserted rather than assumed.

The merge applied program spans after `compiled.phases`, so a span named
`install` or `build` replaced the runner's own measurement. `spawn`, `run`
and `total` are assigned after the spread and were never reachable, which
is exactly the pair the test checked, so it passed while the claim it was
meant to prove was false:

  install: 999999  <- program value
  build  : 888888  <- program value
  run    : 0.0076  <- runner value

The runner's phases are applied last. The test now asserts every owned
name rather than the two that happened to work, and fails on `install`
with the fix reverted.
