Reject cyclic asset controller chains (#1319)

* fix(gatekeeper): reject controller cycles

Track visited controller DIDs during signature verification and reject updates whose proposed controller chain closes a cycle.

* fix(gatekeeper): validate live controller graph

Controller-cycle checks must use the latest accepted graph rather than signature timestamps or confirmation state. Serialize graph writes so concurrent operations cannot commit a cycle, and validate asset creates that complete a latent cycle.
