Agents: per-user turns, queued messages, clearer tool errors (#279)

* Agents: per-user turns, queued messages, clearer tool errors

Turn dedup was keyed (parent, thread, agent) with no invoker, so on a busy
channel one person's live turn made the agent refuse everyone else's mention
— silently, because ErrAgentBusy counts as "already invoked". A run belongs
to its invoker, so the key now does too.

A message arriving mid-turn was dropped for the same reason. The in-flight
run's prompt predates it, so its reply cannot answer it; it is queued now and
starts when the turn ends.

Also fixes two things that only show up with more than one instance (prd runs
two): the typing ticker and the thread-turn slot are per-process, but a run's
life is not, so whichever instance did not handle the terminal transition kept
"… is typing" going forever and held the slot, muting that thread for good.
Both are now checked against the store.

And agent tool errors say what happened: send_dm accepts the [u:<id>] form
list_users hands out, and "could not open the DM" names the id and the next
step. A model told only "could not open the DM" invented workspace
restrictions that do not exist.

* Agents: start a queued turn when another instance finishes the run

Parked handoffs live in per-process memory, but the run's terminal
transition is load balanced: when a sibling completed the run it drained
its own empty map and the queued message was never answered. The holding
instance now watches the run itself.

* Connectors: ask for an access token and say where to get one

"Bearer token" is how the credential rides the request, not what the
user goes and creates. credentialHint/credentialURL come with the
registration so each service words its own instruction; GitLab falls
back to a derived hint until its registration carries one.

* Agents UI: 14px minimum text size

Agents, Skills and Connectors only — text-xs (12px) was below the floor
the CTO set.
