Use multiple decoders for noise learner and client-side primitives (#3393)

### Summary

Actively use multiple decoders for noise learner and the client-side
primitives, via:
* A new method `is_applicable` to `ResultDecoder`, that determines
whether a decoder is to be applied
* Modeling those existing decoders as actual subclasses of
`ResultDecoder`
* client-side: via a `BaseClientSideResultDecoder` that encapsulate the
common patterns they use, and by subclassing it for each program
   * noise learner: via using `is_applicable` as the differentiator
* the default list of decoders now contains lists, and the method that
actually invokes them makes use of `is_applicable` for skipping them

On top of that - renamed `decoders/quantum_program` to
`decoders/executor`. All of them are named based on the program, rather
than the intermediate construct: and this brings more consistency.

### Details and comments

This provides a more clean mapping between a program and its decoder,
encapsulating it (and in particular avoiding the one for `executor`
having to keep track of every client-side implementation). It comes at a
cost of some opacity: we don't have the full programmatic clarify of
dispatching decoders in a function (ie. no `if` or advanced logic, we
rely on the ability of them to be skipped) - but provides a way of
adding decoders seamlessly and get closer to allowing to "hook" them
(now we _just_ need to add them to the defaults, and provide an isolated
implementation, with no changes in other decoders).
