feat!: make -a stage tracked changes only and add -A to stage everything (#2)

The -a/--all flag used to stage every change, untracked files included, which does not match git. It now stages modified and deleted tracked files only, like git commit -a, and fails early when there is no tracked change to commit. The new -A/--add-all flag stages every change, untracked files included, like git add -A. The two flags are mutually exclusive and both conflict with --hook and --stdout.

BREAKING CHANGE: -a/--all no longer stages untracked files. Use -A/--add-all to stage them.
