Update the changelog and 'make cutarelease' style

Previously, 'make cutarelease' would involve doing a couple commits --
to tweak the changelog and bump the version in package.json. While
sometime convenient, it wasn't ideal because: the extra commits were
noise, who says the next release version will just be a patch-level
release, this CHANGES.md style didn't match what node-restify is doing.

The new style:
- there is a top "## not yet released" section in CHANGES.md for
  unreleased/unpublished changes
- 'make cutarelease' will tag and npm publish the current version

To do a release, then, you:
1. change version in package.json
2. add the "## $version" section to CHANGES.md
3. `make cutarelease`
