docs: explain run environment setup and startup (#1166)

## Summary

Explain setup/startup selection and lifecycle in CLI help and the
shipped agent skill, including flat JSON, installation paths, saved
definitions on resume, and preparation failures. Add a checked Node 22
setup example, clarify API nesting, and correct the Docker README and
validate-only help.

## Validation

- Three agents reviewed contract accuracy, coverage, and wording; fixed
API nesting and unnecessary credential requirements.
- Existing CLI help and generated-reference tests: 12 passed; references
regenerated and checked.
- Strict MkDocs build, skill validation, formatting, distribution
checks, and Opcore Verify passed. Markdown received manual review;
Opcore covers the Rust help source.
- Example Bash/JSON validation and Node archive checksum/executable
smoke passed; the local smoke stubbed apt.
