Give a nested stringref namespace its own index space (#335)

A tag 256 value nested inside another starts a fresh index space, so the outer string and bytestring reference tables are set aside for the duration of the tagged value and restored afterwards. Previously the emitted references indexed the outer namespace while a decoder resolved them against the inner one.
