feat: engine-agnostic HighlightedCode renderer with Shiki and TanStack Highlight engines (#394)

Extract the two-method CodeHighlighter contract, the HighlightedCode
renderer, the context key, and the singleton into an engine-free
extensions/highlight subpath. Shiki becomes a factory on its own subpath
with every previous export kept as an alias. Add a second factory for
@tanstack/highlight (~4 KB gzip for three languages vs ~87 KB for Shiki),
wired to its synchronous API with escaped fallbacks and adversarial-lang
coverage.

Both engines are optional peers; the core bundle, the extensions barrel,
and the shared renderer stay engine-free, and the two engines never leak
into each other (barrel test + five new tree-shaking cases).

Docs: rewritten Syntax Highlighting page, tree-shaking table, README, a
side-by-side streaming demo with per-engine timings, and a blog post
draft with full working setup for each engine.

Visible change: with no highlighter configured the fallback <pre> now
carries highlight-fallback instead of shiki-fallback.
