Rewrite sessions towards a more usable API

Rebuild the package from the ground up to improve usability. Most APIs
have changed, and sessions created by v0.4.1 cannot be read by v0.5.0.

- The Manager is built from three separate parts: a Transport (cookie
  or bearer), an Encoder (HMAC, AES-GCM, or rotating keys), and a Store
  (cookie or file).
- Sessions are never created or saved automatically. Load, New, and
  Save are explicit, and every route that uses sessions needs
  Manager.Middleware().
- Cookies are Secure by default, and store errors are no longer
  reported as a missing session.
- Flash, Codec, custom serializers, and the context helpers have been
  removed.
