perf: Lowercase in chunks when '<' is dense

A bytes.IndexByte call per '<' before </head> costs about 9ns, which
makes a head dense in tags slower than lowercasing the whole page.
Past one failed candidate per 16 bytes, indexFoldASCII lowercases the
rest in 2 KB chunks on the stack and searches each with bytes.Index,
whose cost doesn't depend on the density.

BenchmarkMiddleware, Apple M4 Pro, benchstat medians:

  140 KB of <meta/> tags   195us  -> 128us
  100 KB of <              813us  -> 86us
  4 KB page                2.34us -> 2.37us
  80 KB head               26.3us -> 27.7us, not significant
