fix: keep vanilla deploys independent of pending partial uploads

A vanilla deploy of an entity with a live pending upload inherited that
upload's created_at as its TTL anchor, so a full deployment could skip
the normal freshness check. It also wrote a completion receipt from the
pending row's deployer, so when another authorized signer published the
entity, the partial uploader's next batch got a false 200 completion.

The vanilla path now always validates against now; only partial staging
and finalization anchor on the pending row. Completion receipts are
written only when a partial finalization publishes the entity, and are
recorded for the finalizing signer. Any publication still drops the
pending row in the same transaction, so its accounting never leaks, and
the redundant post-deploy cleanup in the handler is gone. Staging checks
whether the entity is already deployed before validation, so a batch for
a superseded upload gets "already deployed" instead of a stale TTL error.
