fix(interact): read plain backend input on demand

plain.Session wrapped the input stream in a per-session bufio.Reader and ran
an eager reader goroutine that pulled every line into a private channel.
ReadEvent consumed a single line, but the lines already read stayed trapped in
the session that was ending, so any later prompt on the same stream (pipe or
redirected stdin) only saw EOF.

Read one line per call instead, byte by byte, so bytes after the requested line
stay in the stream. At most one read is in flight per session and a call
cancelled by ctx resumes that read instead of abandoning it, so no line is
dropped and no second reader races the stream. A read after the input ends now
reports the error again instead of blocking on an empty channel.

Verified: the interact-ui demo with redirected stdin died at the second prompt
with EOF before the change and completes all four prompts after it.
