Add base conversion: hex/oct/bin functions and 0x/0o/0b literals

Adds hex(n)/oct(n)/bin(n) to format a non-negative integer as a
0x/0o/0b-prefixed string, and 0x/0o/0b-prefixed numeric literals to parse
hex/octal/binary numbers as ordinary values usable anywhere in an
expression (arithmetic, variables, function arguments, mixed bases).

This introduces the crate's first non-numeric result type: eval() now
returns a Value enum (Number/Text), with a new evaluate_value() entry
point in lib.rs. The existing evaluate() (f64) API is preserved unchanged
for backward compatibility, erroring with NotANumber if the result is
text. excalc-mcp and the CLI now use evaluate_value() so hex/oct/bin
output works end-to-end.

Adds docs/functions/base-conversion.md, README examples, and extends
tests/readme_examples.rs to support string-typed expected values.
