docs: add project AGENTS.md for agentic session context

Documents the module map, entry points, the Schematron public API,
the empirically verified elementpath gotchas (document-node semantics,
wrapped node returns, sourceline absence) and project conventions
(bare-version tags, version sync, make check as the gate) — so future
sessions start from facts instead of re-reading the code.
