feat: create a GitHub release after each npm publish

Adds a step to publish-to-npm.yml that runs gh release create with
--generate-notes once npm publish succeeds, so every tagged release
gets a GitHub Releases entry with auto-generated notes instead of
just a tag. Needs contents: write for the default GITHUB_TOKEN to be
allowed to create it.
