Skipping TSIG on zone transfers is meaningless when zone transfers are not
enabled, so reject the combination at startup rather than silently ignoring it.

--rfc2136-tsig-axfr also satisfies this: resolveDeprecatedFlags folds it into
--rfc2136-axfr before ValidateConfig runs.

* test(rfc2136): assert AXFR requests honour --rfc2136-insecure-axfr

Capture the message the stub receives so the test checks the TSIG record on the wire, not just the predicate that decides whether to attach it.

* docs(rfc2136): note insecure-axfr now requires --rfc2136-axfr

* refactor(rfc2136): rename --rfc2136-insecure-axfr to --rfc2136-axfr-insecure
