Implement Modelica document and range formatting (#86)

* Implement Modelica document and range formatting

Add syntax-aware whitespace formatting with two-space defaults, preserve
comments and strings, and format the current unsaved document contents.
Cover formatting with server, LSP transport, and VS Code regression tests.

* Expand formatting coverage for Modelica language constructs

Add exact-layout cases and token/syntax preservation checks for redeclarations,
external functions, imports, connectors, arrays, tuple assignments and control
flow. Verify repeated formatting with spaces, tabs and narrow line widths,
plus escaped documentation, interleaved comments and selection boundaries.

Show the Command Palette workflow and two-space indentation result, with
instructions for workspace trust, formatter selection and range formatting.

* Verify embedded XML documentation remains untouched by formatting

Cover XML declarations, CDATA, entities, escaped attributes, internal
whitespace and LF/CRLF in document and range formatting. Also verify
malformed XML/HTML is preserved and document the behavior in the README.

Keep embedded markup unchanged by default, expose startup/live/request
configuration, and delegate opted-in documentation to markup formatters.
Cover preservation, range boundaries, escaping, stability and VS Code toggles.
