doc: record the build_state flip, and that its blocker had expired

Phase 7 step 2a concluded that flipping `build_state` was `from_states`-bound and had
to wait for step 3. All three premises had since lapsed -- `ManyBodyState` is the
unified block class, the Lanczos kernel grew a block-seed path, and returning one WIDE
block (rather than a list of width-1 blocks, which is what that attempt tried) makes
the three biggest consumers simpler instead of harder.

The note recording the old verdict was still on the function, asserting "not yet
flipped to width-1 ManyBodyState" against code that plainly built one. A stale decision
note reads exactly like a live constraint, so this records the expiry as loudly as the
change.

Also records the defect class the new contract carries -- `len()` on a block is its
rank-local row count, not its width -- and the aliasing hazard that any
build-a-new-container -> pass-the-caller's-object flip inherits, which produced a wrong
ground-state energy with no exception and was found by review rather than by a green gate.
