Release prep for 4.1.0: release notes and README fix (#32)

* Release prep for 4.1.0: release notes, README proxy_list wording

- Add RELEASE_NOTES_v4.1.0.md covering everything since 3.0.0 (4.0.0 was
  never published), with an upgrade section up front.
- Remove the unpublished 4.0.0 draft.
- README option table: a complete IP proxy_list entry is an exact match.

* CI: publish with skip-existing so a tag run stays green after a manual upload
