test(mcp): split server tests into focused suites (#443)

## Summary

Split MCP server tests into eight feature-group suites with a shared
lifecycle harness and selective healthy-project fixture. Preserve all
187 server cases and add contributor guidance.

## Verification

Passed locally from the repository root:

```bash
pnpm --filter @supabase/mcp-server-supabase test:unit --run # 408 passed
pnpm --filter @supabase/mcp-server-supabase test:unit --run src/server # 187 passed
pnpm --filter @supabase/mcp-server-supabase typecheck
```

Full external coverage (`pnpm run test:coverage`) was **not run**
because existing Anthropic authorization is missing.

651 of 692 branches covered (94.08%)

Branch coverage included in aggregate %.

4072 of 4174 relevant lines covered (97.56%)
