Resolve schema-typed element namespaces despite key conversion

Elements whose types live in another namespace received the tns: prefix instead of their own prefix, and strict services rejected those messages.

Root cause: QualifiedMessage finds a type namespace by message path, but message keys pass through the key converter first (:entity_ids becomes "entityIds" while the schema defines "EntityIds"). The lookups missed, so every element fell back to tns:.

Fix: index the type and namespace tables by downcased path. When an exact lookup misses, try a case-insensitive match. An exact match always wins.

Fixes #895.
