fix: map three missing-row cases to not found or unauthenticated

A domain that does not exist came back from the repository as a wrapped
database error, so get, verify, and delete answered with an internal error.
The repository now returns the domain not-found sentinel the handlers already
map.

CreatePolicy with a role id that does not exist fell through to an internal
error. The handler now maps it to not found.

A JWT grant whose credential points at a service user that no longer exists
returned the raw error from the authenticator. It is now unauthenticated,
the same as a grant that fails verification.
