breaking: include full header in notify instead of hash and num (#195)

#### Description

The block_hash and prev_block_hash naming was confusing. After
discussing
with the committer team, we decided to replace the block_number,
block_hash,
previous_block_hash with a common.Header (block number, data hash,
previous
block hash). The header hash can be calculated by the client using
existing tools
(Fabric protoutil).

This is a cleaner, more standard approach that is easier to understand
for the user.
