ci: publish test coverage and report-card links (#26)

## Summary
- The existing coverage recipe now writes `coverage.out` from `go test
-coverprofile`, and CI uploads that file to Coveralls with
`secrets.GITHUB_TOKEN` only.
- The README badge row keeps pkg.go.dev and adds the Go Report Card and
Coveralls report links.

Statement coverage, from `go tool cover -func` on that profile (`go
test` of `./pkg/claude/...`), is 80.5%. No new tests.

Go Report Card grade could not be fetched. The service was sunset on
2026-07-01, and
https://goreportcard.com/badge/github.com/lancekrogers/claude-code-go
now reads "retired".

## Checks
- `just coverage percent` wrote `coverage.out` and reported `total:
(statements) 80.5%` (`pkg/claude` 79.6%, `pkg/claude/dangerous` 94.8%).
- `just test dangerous` passed, and `coverage.out` was still present
afterward.
