* jit: re-use a module already imported in this process

Every cache hit imported the extension module afresh: a
module_from_spec/exec_module round trip plus several stats, for a module
already mapped into the process. A module name is a signature over the
forms, the options and the compilation flags, so a repeat lookup is by
construction the same generated code.

Memoise the loaded module on (module name, cache directory). The
directory is part of the key so that compiling the same forms into a
second cache still loads from that cache, and only a caller-supplied
cache directory is recorded, so the per-call temporary directory used
when no cache is configured does not accumulate entries.

_load_objects() also located the module with the directory-scanning
finder that get_cached_module() no longer uses; it now shares the same
path-based helper and populates the memo, so the saving lands on the
first repeat rather than the second.

Measured on fem.form() against a 24,196-file cache, interleaved over
three rounds, best of each:

  #887                  0.115 ms
  #887 + this           0.086 ms
  #887 + #888           0.066 ms
  #887 + #888 + this    0.034 ms
