This was cleanup from prior PR. cleanup lint and test fixtures (#26)

* chore: clean up lint and test fixtures

* ci: update GitHub Actions to supported major versions

* fix(ci): skip Coveralls upload on fork PRs

Secrets aren't exposed to fork PR workflows, so the coverage
step failed with "Missing Repo Token". Now the step only runs
for pushes and same-repo PRs.
