Add writing rules for commits, PRs and comments to CLAUDE.md (#978)

Recent PR descriptions were accurate but hard to read. Sentences held
three to five facts joined by "so", "which" and semicolons. Code and
domain concepts were paraphrased instead of named.

- Add a prose rule at the top of Conventions that covers changelog
entries, comments, commits and PRs: one fact per sentence, name
  identifiers and domain terms directly.
- Add a "Commits and pull requests" section: summary first, a fixed
layout for multi-part changes, mechanism written from the diff,
what to leave out, formatting, and a worked example.
- Say that the changelog's effect-not-implementation rule does not
  apply to PRs.
- Split the densest paragraphs elsewhere in the file. No content
  changes.
