Release 3.4.0 (#579)

Prepare SQLiteCpp 3.4.0 for release on 2026 Sep 21.

- bump all project version metadata to 3.4.0
- finalize the 3.4.0 CHANGELOG
- document 3.4.x as the final C++11 release line and `sqlitecpp-3.x` as
its maintenance branch

After merge, tag `3.4.0`, publish the GitHub release, then fast-forward
`sqlitecpp-3.x` to the release point before starting 4.x work on
`master`.
