Add code coverage reporting to Coveralls

The Build and Test workflow now collects coverage with XPlat Code Coverage
(coverlet) and uploads Cobertura reports to Coveralls using the built-in
GITHUB_TOKEN. No account setup or repo secret is needed. The test projects
both reference coverlet already; CodeCoverage.runsettings filters the results
to the shipping library only.

A coverage badge was added to the README between the Build Status and NuGet
badges.

Also fixed: ArgumentParser.sln's Solution Items "workflows" folder now lists
build-and-test.yml and release.yml instead of the deleted ci.yml.
