ci: run the tests on pull requests, against the ref under test

The workflow only ran on pushes to master, and its checkout step was
pinned to master, so even a manual run on another branch tested master.
Pull requests now trigger it, and the checkout takes the ref that
triggered the run.

Coverage moves into a job of its own which runs after the tests, for
master only, which is all the readme badge reads; pull requests from
forks could not publish it anyway, as their token is read-only. Both
jobs cache the npm dependencies, and checkout and setup-node move to
their current major versions.
