forms: render horizontal buttons as Bootstrap 4 (#897)

{% buttons layout="horizontal" %} produced Bootstrap 3 markup, so the
submit button sat out of alignment with the horizontal fields above it.

render_field_and_label added control-label, which Bootstrap 4 renamed to
col-form-label and which therefore carried no styling at all. The wrapper
was a plain form-group with no row, leaving col-md-3 and col-md-9 with no
flex row to divide. FieldRenderer gets both of these right; this path
through ButtonsNode is separate and was never updated for Bootstrap 4.

Output now matches what a horizontal field renders. The default layout is
untouched, since neither the row nor the label applies there.
