Restore canonical README structure and fix the example-app command (#1150)

PACKAGING.md names django-bootstrap3 and django-bootstrap5 as the canonical
reference for README structure, but this README was the only one of the five
still using setext underlines for its last four sections. Convert them to the
`##` headings every sibling uses.

The example-app section also pointed at `make example`. There has been no
Makefile here since the move to just; the command is `just example`.
