Prevent overflow in snapshot durations

Clamp negative ages when a concurrent update publishes a timestamp
after the snapshot clock reading. This prevents saturated expiration
and refresh metadata from wrapping to a negative duration.

Add concurrent replacement and clock-wrap regressions, and clarify the
snapshot caller's lock ownership for parallel traversal.
